Healthy rEQuest is your free wellness program at EMBARQ. You can count on Healthy rEQuest for help, information and advice — log on or call as often as you need. Healthy rEQuest is provided as a benefit by EMBARQ at no cost to you and is completely confidential.

No one at EMBARQ will ever see your personal health information.
 

Healthy rEQuest is administered by independent health management companies — SHPS and StayWell — and is 100% confidential.

Most elements of Healthy rEQuest are provided to all EMBARQ employees, eligible retirees and COBRA participants, but depending on where you work, your job arrangements and other factors, not all of these services may be available to you. EMBARQ intends to continue to provide this coverage; however, EMBARQ reserves the right to change or discontinue any or all benefits at any time without notice. In the event that a benefit is discontinued, EMBARQ will notify participants as soon as reasonable possible.

Want a discount off the cost of your Embarq-sponsored
medical coverage for 2009?

As an active employee, you are eligible to receive a discount for completing your Health Assessment. Once you (not your spouse or qualified domestic partner) take your personal Health Assessment, EMBARQ will be notified of the completion only and you will receive a discount of $250 off the annual cost of your EMBARQ-sponsored medical coverage when you enroll for 2009. You do not have to get a health screening to get the discount. All you have to do is complete the Health Assessment.
